Our suite of services (CCM,PCM,RPM,TCM,BHI,AWV) revolve around a unique team of healthcare professionals who have skills and attributes that compliment health center Providers and supporting staff. The Vigilance partnership model provides a remote clinical care team of Nurse Care Managers, Pharmacists, Dieticians and Health Coaches that work as an extension to the health center team. The care team acts as a force multiplier to Providers to keep the patient engaged and adherent to their directives.
