Session Description: Remote Patient Monitoring (RPM) is a growing way of delivering care, with providers from primary care to specialists seeing the benefits of monitoring their patients' health in between clinic visits. Until 2024, Federally Qualified Health Centers and Rural Health Centers were not eligible for reimbursement for RPM services and relied on grant opportunities to build RPM programs.
In this session, we will share how La Clínica del Pueblo worked with Rimidi to implement an RPM program for patients with diabetes and hypertension to improve patient outcomes, optimize workforce efficiency and grant dollars, and how they plan to sustain and grow the program.

Understand RPM basics, including the current reimbursement landscape for CHCs and FQHCs.
