Session Description: Practice transformation to prepare for the future of medicine feels daunting, but it does not have to be. Learn how to transform your practice today with minimal effort, extra support for patients, and additional revenue streams. In this presentation, we will share real-world examples of value-based care delivery – from “no-risk” quality improvement models to “high-risk” capitation models (and everything in between). Additional topics discussed will include quality incentives and Accountable Care Organization (ACO) models that are aimed at improving quality and reducing costs—pitfalls and pearls will be addressed. We will also share practical methods to enhance patient engagement and patient experience with no extra burden to existing care teams. Finally, we will discuss how operating in a value-based care environment can provide you with access to expanded care teams to assist with addressing Social Drivers of Health.
