Session Description: In 2023 the Kentucky Primary Care Association and Clinically Integrated Network invested in a new data analytics platform with the goal to streamline population health reporting and analysis and advance value-based care initiatives across the network. This investment required developing a strategic framework to engage the 29 member community health centers and 55 rural health centers in leveraging technology in service of a unified set of goals – for KPCA, where both FQHCs and RHCs are members, this offers unique opportunities to impact thousands of patient lives through population health analytics and value-based care advancement. The PCA pursued a strategy to integrate EHR, HIE, and enrollment and care gap data into one platform, offering the network and members a single platform for day-to-day population health management and analysis. They also invested in a cost and utilization tool to track spending, ED and inpatient activity, and progress toward plan targets, aggregated across their payer contracts. In this session, hear from the Kentucky PCA and Clinically Integrated Network about how they are pursuing their value-based care goals and supporting members through an integrated data analytics platform, and how other networks can do the same.
