Gabriela Villalobos, BS, BA, serves as the Director of Care Management at Settlement Health, where she is dedicated to raising awareness of racial and ethnic health disparities. With nine years of experience at Federally Qualified Health Centers in New York and California, Gabriela has led numerous population health initiatives, including transitions of care, patient risk stratification, Team-Based Care initiatives, and efforts to close gaps in chronic disease management. Her current projects concentrate on optimizing health information technology to enhance clinical and social determinants of health outcomes.
